This section contains the guidelines for Local Coordinators reference only. These materials are restricted documents and should not be shared with the Experts or other unauthorised parties.

  • This document contains guidelines to host the Visiting Experts. Local and Main Coordinators are encouraged to read through the whole document.

Administrative Documents (Pre-Visit and Post-Visit)

This section contains the administrative documents required by Local Coordinators and Visiting Experts to plan and prepare for the visit. These documents are listed in chronological order of usage. Refer to administration manual for submission timelines.

Pre-Visit

• Form 1 - Programme Template 2025 [DOCX, 41.8 KB]

  • Template for programme submission. Local Coordinators to ensure that the Experts endorse the cover of the programme.

• Form 3 - FY25 Budget and Entitlement [XLSX, 50.5 KB]

  • Document to assist Local Coordinators in planning the budget and entitlements. LCs must submit this document with the programme to seek MOH's approval.

Post Visit

• Form 2A - FY25 HMDP Report - Expert [DOCX, 46.3 KB]

  • Template for report to be submitted to MOH

• Form 2B - FY25 Report - LC

  • Template for report to be submitted to MOH. To be submitted together with Form 2A.
