Guide to MediSave usage for chronic disease management.
The Chronic Disease Management Programme (CDMP) was first introduced in 2006 to help patients with chronic conditions to reduce out-of-pocket expenses for outpatient management of their conditions and help to promote health-seeking behaviours. Eligible patients can utilise their MediSave at MediSave-accredited healthcare entities (all polyclinics, all public hospitals’ specialist outpatient clinics, accredited specialist clinics, and accredited GP clinics) and subsidies at CHAS GP clinics for their chronic conditions.
These conditions often lead to more serious complications if detected late or not followed up regularly. Individuals are therefore encouraged to have a regular family doctor, undergo the recommended health screenings and have regular reviews of their chronic condition, timely medical treatment and lifestyle changes.
The CDMP currently covers a total of 23 chronic conditions:
Diabetes Mellitus / Pre-diabetes
Hypertension
Hyperlipidemia (Lipid Disorders)
Asthma
Chronic Obstructive Pulmonary Disease (COPD)
Chronic Kidney Disease (Nephritis / Nephrosis)
Schizophrenia
Major Depression
Bipolar Disorder
Anxiety
Stroke
Dementia
Osteoarthritis
Parkinson's Disease
Benign Prostatic Hyperplasia
Epilepsy
Osteoporosis
Psoriasis
Rheumatoid Arthritis
Ischaemic Heart Disease

Under the MediSave 500/700 scheme, patients can use up to $700 per year if they have received treatment for two or more CDMP conditions in a visit or received treatment for at least one CDMP condition with a recognised complication, or $500 per year otherwise. Each claim is subject to a 15% co-payment in cash. Since 1 February 2024, the 15% co-payment is waived for patients seeking CDMP treatment at their enrolled Healthier SG clinics.

The CDMP Handbook for Healthcare Professionals (“CDMP Handbook”) is regularly updated to serve as a quick guide on CDMP and cover a range of practice issues from administrative and claims-related matters to broad guidelines on recommended care components of clinical care.
Updated 10 September 2025:
From 1 September 2025, vaccinations recommended under the National Adult Immunisation Schedule (NAIS) will be termed “Healthier SG Vaccinations” and vaccinations recommended under the National Childhood Immunisation Schedule (NCIS) and recommended childhood developmental screenings will be termed “Childhood Developmental Screenings and Childhood Vaccinations”.
This renaming does not change the name, structure and scope of the existing NCIS and NAIS. There are also no changes to the benefits provided to both Healthier SG enrollees and non-enrollees under VCDSS today. Participating CHAS GPs (which may or may not be a Healthier SG clinic) can continue to offer the same subsidised NCIS/NAIS vaccinations and childhood developmental screenings previously provided under VCDSS. Eligible Singapore Citizens will continue to enjoy subsidies with no change in benefits or subsidy quantum. In addition, eligible Singapore Citizens enrolled in Healthier SG will continue to enjoy special subsidies for selected Healthier SG vaccinations at their enrolled Healthier SG clinic.
The CDMP Handbook has been updated to reflect the changes made to Page 28, 40, 43, 45, 46, 52, 54, 58, 62, 73, 81, 83, 91.
